Parents should complete the application and submit the form and supporting documentation to the South County Sports Club, PO Box 1655, Lorton VA 22199.       Virginia DMV ID Card Information          

 

Virginia DMV ID Card Information  If you are applying for an ID card for the first time and do not hold a valid Virginia driver's license, each time you apply for the ID card, you will be required to show two proofs of identity (only one primary document if you are under age 19), one proof of legal presence and one proof of Virginia residency. You will be required to show these required documents on each return visit until your ID card is issued.

Adobe PDF fileVBD School Property Damage Report * - Completed only when reporting damage to school property and submitted by fax to Fairfax County DCRS within 24 hours of the incident. Indicate the who, what, where, when, and why. Information concerning the person(s) responsible for the damage will enable Fairfax County DCRS to seek financial reimbursement for the cost of repairs. Provide a specific and thorough description of the damaged item and of the damage. Specify the location of the damage (e.g., girl's restroom in the gym).

Adobe PDF fileVBD Bi-Weekly Report * - The first Coach to use each gym does the first inspection and the last Coach to use each gym does the last inspection. Indicate the day and time of gym usage. Record the time of facility inspection by building custodian before and after gym use. Briefly describe damage to facility during use, if any. If no damage, indicate so. Do not leave this space blank. Validate report with signatures from VBD and building custodian.   

Adobe PDF fileVBD Accident Report * - This is not an insurance claim form. Fairfax County DCRS must document injuries sustained during community use activities in the event of a legal dispute. Indicate the who, what, where, when, and why. Be specific and thorough in completing the report (e.g., front of the left thigh). Provide contact information for at least two witnesses. Fax and mail the report to Fairfax County DCRS within 24 hours of the incident.
